- If you were to get sick during the trip, we would have access to your current medications(both presciptions and over-the-counter) for quick access, and have information of any allergies.

- If you have no medical conditions to list, then simply indidate "none".

- As you know, TRAVEL INSURANCE is required. You must provide proof of medical coverage, and can indicate it on this form. (It is too late for group coverage at this time, but if you have not purchased your medical coverage yet, you can search online at www.insuremytrip.com for some comparison rates.
